| We proudly offer a full array of services
grounded in best practices for all ages, genders, disabilities, and cultural diversities.
Our consumers are empowered as the leader of the team steering the treatment. Services are
built in collaboration based on the consumer's desired goals, utilizing the principles of
consumer choice, individualization, person centered, consumer driven, strengths-based
treatment. We consistently strive to address the
unique needs of each individual, implement evidence based practices, and utilize QA/QI
measures to affect the best possible outcomes. |

| Diagnostic/Outpatient Assessment |
|
|
- A face-to-face, clinical evaluation of a consumer's
behavioral health needs. The assessment provides valuable information that allows us to
effectively match needs with services. New consumers, children and adults are encouraged
to have a comprehensive clinical assessment prior to person centered plan development.

| Psychiatric Services/Medication

- This service is available to child, adolescent, and adult
consumers that require medication to manage their mental illness. TBH contracts, employs,
and collaborates with licensed medical doctors and nurses to provide this service.

| Outpatient Therapy |
|
|
- Outpatient treatment is designed to address the clinically
significant behavioral and/or psychological symptoms of the consumer and family or support
system. This service is provided through scheduled therapeutic treatment sessions. The
service may be provided to an individual, couples, families or groups.

| Child Community Support |
|
|
- The focus of treatment is on consumers developing skill
building techniques to accomplish goals toward independence. The model involves a
qualified professional who maintains oversight of the Person Centered Planning process and
provides and oversees case management to arrange, link, monitor and/or integrate multiple
services. Associate professionals and/or paraprofessionals provide assistance with
therapeutic interventions to increase the consumer's acquisition of skills needed to
accomplish goals on their Person Centered Plan.

| Adult Community Support |
|
|
- The focus of treatment is on consumers developing skill

| Community Support Team |
|
|
- This service is provided by a small intensive team of
professionals and paraprofessionals working with consumers to assist the individual in
acquiring the necessary skills to successfully manage their illness and recovery in the
least restrictive setting possible. Services are primarily provided in the community,
working with consumers to manage housing/finances, medication compliance, vocational
goals, and counseling needs on an individual basis for successful community living.

| Psychosocial Rehabilitation /
Supportive Employment / Vocational Rehabilitation (CounterPoint) |

- A day treatment program that allows adult mentally ill and
dually diagnosed consumers to engage in a communitv and within the community and
participate in skill development activities, socialization with others, behavioral
management, psychiatric symptom management, life skills development to support educational
progress and pre-vocational training.

| Grand Oaks - Supervised Apartments |
|
|
- 10 supervised apartments designed to treat highly
independent, mentally ill adult consumers that choose to live in a community setting.

| Child/Adolescent Residential Level
III |

- Serving female and male children ages 7 18 years that
need mental health services in a highly structured and supervised 24/7/365 environment.

| Assertive Community Treatment Team
(ACTT) |

- An intensive, interdisciplinary, self contained outreach
team that provides intensive individualized services to adult mentally ill and
dually-diagnosed consumers in the community.
