Responsible for providing overall coordination and oversight of a
caseload in the Shelby Community Support division, which delivers professional behavioral
health services to mental health consumers. This shall include, but not be limited to:
Coordination and oversight of initial and ongoing assessment activities; initial
development and ongoing revision of Person-Centered Plans; and additional case management
functions of linking, arranging for services, and referrals. Services may be both office
and community based. The individual utilizes professional knowledge, skills and abilities
to deliver services that address the needs of consumers, organization, purchasers of
service, and accrediting/ licensing bodies.
Minimum of a bachelor degree in human service field from an
accredited college/university.
At least two (2) year full-time
post-bachelors degree accumulated mh/dd/sa experience with the population served in
delivering clinical services. (Meet NC
Qualified Professional (QP) Status 10A NCAC 27G.0104.)
Bachelors degree in a field
other than human service and has four (4) years full-time post-bachelor experience
accumulated mh/dd/sa experience with the population served.
Masters degree in human service
field from an accredited college/university with at least one (1) full year of
post-graduate degree in accumulated mh/dd/sa experience with the population served.
Must maintain credentialing and
privileging for Qualified Professional.
